Erik  Bork  PT,DPT, OCS


Erik is described as a mechanist in his therapeutic technique. He closely studies joint mobility and mechanics, and performs treatments that combine a strong manual therapy approach along with exercise prescription and neuromuscular approaches to restore normal movement. Erik emphasizes strength and balance, specifically with older adults. Through the restoration of body strength and balance, there is less load and force on the joints, ligaments, and afflicted tendons. Erik has extensive education in the evaluation and treatment of shoulder problems. He has taken numerous continuing education courses with the specific emphasis on the shoulder region and covering all types of injuries and problems related to shoulder function. He is experienced in non-surgical and post-surgical rehabilitation. Erik has worked extensively with baseball players and other overhead athletes in high school, college, and beyond. In his career, he has worked with patients from 6-99. Erik received his Bachelor of Science in Biology at Concordia University in 2000. He completed his Master's of Physical Therapy in 2002 and Doctorate of Physical Therapy in 2003.

Jamie grew up in Rice Lake, WI and earned her B.S. in Exercise and Sports Science in 2012 and her Doctorate of Physical Therapy in 2015, both from UW-La Crosse.  While in La Crosse Jamie competed 4 years on UW-L’s collegiate gymnastics team, and enjoyed staying involved with the sport through coaching. Following graduation, Jamie worked in an outpatient physical therapy clinic in Rhinelander, WI, where she became certified in dry needling through Myopain and in Applied Functional Science through the Gray Institute.  Jamie joined the Integra team in the summer of 2016 and looks forward to increasing her involvement in Wisconsin Physical Therapy Association programming.   During her free time Jamie enjoys spending time on the lakes, hanging out with family and friends, running, hiking, rollerblading, cheering on the Packers, and playing Pinochle.

Jamie Hofschulte PT,DPT

Stef enjoys her work as a Physical Therapist and is currently involved in a 2 year Orthopedic Development Program through the Wisconsin Physical Therapy Association. This program emphasizes further expansion of manual skills and exercise based treatment along with discussion of current evidence pertaining to the diagnosis and injuries commonly seen here at Integra. She earned her BS from UW-La Crosse in 2010 and her Doctorate of Physical Therapy from Concordia University in 2013. After graduate school, Stef worked at the outpatient clinic of Beaver Dam Community Hospital. She moved back to Milwaukee in the summer of 2014 to join our Integra team and has become involved with the SE District of the Wisconsin Physical Therapy Association.

Stef Smith PT,DPT, OCS

Jeremiah works as a physical therapist and also has experience in personal training and athletic development. He earned his Bachelors of Science in Kinesiology with a Human Performance emphasis from the University of Wisconsin-Eau Claire in 2008. While at Eau Claire, Jeremiah was a member of the men's varsity hockey team receiving honors as a 4-year member of the NCHA all academic team, nominated for NCHA scholar athlete of the year, as well as a two-time recipient of the Richard G O’brien award for outstanding work ethic. After completion of his BS in Kinesiology, Jeremiah received his Doctorate of physical therapy from Concordia University of Wisconsin in 2011. While at Concordia and post graduation, Jeremiah has pursued specialized coursework emphasizing evidenced based spinal care. Jeremiah is active in the American Physical Therapy /Wisconsin Physical Therapy association and currently serves as the academic liaison Chair for the Southeast district.
